Some stems can be monstrose, others cristate. The size of the stems varies with the season; they are roughly 0.5 inches in diameter during the dormant stage in the winter and frequently develop to double that size in the summer.

With a few tiny hairs poking out of the tiny areoles, they are often smooth.

This cultivar is generally stable, but it does occasionally produce normal offsets from the clump. 

When it comes to flowers, Mammillaria bocasana 'Fred' produces small, dainty blooms in shades of pink or white. The flowers emerge from the top of the cactus and add a lovely touch to its overall aesthetic.
